What do you do if a 1 and a half year old child is lethargic, has hoarsness and has a fever of 103 degrees F.

Sounds like strep. Rapid strep test and augmentin?

I was thinking about that, but hoarseness and wheezing are noticeably different. acute epiglottitis would have to be in your differential, but without hearing the kid...

sounds like acute laryngitis, viral cause is more common than bacteria. Antibiotics wont be effective, increase hydration, give analgesics.

The phrase lethargic always bothers me. I actually would screen for meningitis and then possibly do a tap. I have always been told a sleepy baby gets rest, a lethargic baby gets tapped.
