A 68yr-old woman has had fatigue for 3 months> she takes naproxen for osteoarthritis; digoxin, enalapril, and hydrochorothiazide for artery disease; and amoxicillin for recurrent bronchitis. Her serum urea nigtrogen(BUN)level is 60 mg/dL, and serum creatinine level is 6 mg/dL. Which of the following medications is most likely responsible for her renal failure?
A. Amoxicillin
B. digoxin
C. Enalapril
D. Hydrochorothiazide
E. Naproxen

NSAIDS are a common cause of intersticial nephrtis or renal failure.

my answer is (e)

I'll go with e also. One question, how lond patient has to be on NSAID to get the side effect of nephritis?

there has been no studies for that, because tha would be unethical, but it is well demonstrated that long time use is related to this diseases.

those conclusions have been given by FDA phase "4" cilnical trials. and as an adverse reaction during prospective studies, where they where tested for their analgesic function in the 90īs.

I picked E also.
Don't remember digoxin has a problem with the kidney. I looked up step1 pharmacology, they just say digoxin is cleared by kidney, but didn't find renal failure
