dissection of the aorta ,patients usually presents with pain as the initial presenting symptom

pain is the first presenting symptom of a dissection aneurysm of the descending aorta because it occurs distal to the origins of the brachiocephalic, subclavian, and carotid arteries. you wouldn't get any arm or head symptoms if the aneurysm is in this area.

a, b. This is descending, if anything we'll have increased pressure in both arms and carotid arteries.

c. Seen in severe ascending dissection

d. This is a murmur of aortic regurgitation, seen in asceding dissections but not in descending ones.

e. Severe pain is the most common presentation of all types of aortic dissection. Described as tearing or ripping and increasing in time.
