35. A 32-year-old man has had malaise, a low-grade fever, and a 4.5-kg (10-lb) weight loss over the past 3 months. He has unilateral cervical lymphadenopathy. Laboratory studies show:

Hemoglobin 11 g/dL
Hematocrit 33%
Mean corpuscular volume 80 μm3
Reticulocyte count 0.5%
Serum iron 25 μg/dL
Total iron-binding capacity 150 μg/dL (N=250–460)

Examination of tissue obtained on biopsy of a lymph node shows Hodgkin's disease, nodular sclerosing type. The most likely cause of the anemia is

A) abnormal utilization of iron

B) extravascular hemolysis

C) folate deficiency

D) iron deficiency

E) marrow aplasia

whenever there is chronic inflmmation iron utilization is reduced, thus iron stores are locked away in macrpphages and body is not able tyo utilize it optimally!

now, free serum iron is the active form, which will be low in this instance and because of tht, percent saturation will be low too!

but as the iron stores are increased, so will the serum ferritin level, which will reduce the production of transferrin and therefore TIBC will be low.

choice A is correct!
