Testicular Torsion is best diagnosed by :

1.MRI
2.X-Ray
3.Doppler Ultrasound
4.Radionuclide scanning
5.Venography
6.Physical examination
7.clinical symptoms

testicular torsion is a clinical diagnosis. (7) is my choice

but, i´m a little confused with (6), but...7 is a better guess

The question was : Test. tor. is BEST diagnosed by: it means THE BEST, means the test having highest sensitivity. It is not that how would you suspect, or what wiould be the initial diagnostic test.
Answer is the most sensitive test for test. torsion is Radionucleide scanning, doppler USG comes 2nd as per sensitivity, study_ing please note this.
USMLE fools us like this and we all tend to fell in the trap despite knowing the answer,

radionuclide scan of the testicles can be helpful to assess blood flow and to differentiate torsion from other conditions.
Scan is abnormal in torsion when it demonstrates decreased uptake in the affected testicle, suggesting no blood flow to that side.
Radionuclide scans have a sensitivity of 90-100% accuracy in detecting testicular blood flow.
Color Doppler has a sensitivity of 86%, specificity of 100%, and accuracy of 97% in the diagnosis of testicular torsion when the presence of identifiable intratesticular flow is the sole criterion for diagnosis.

vow sunny 2..thanks a lot...though im early in my prep and tot radionucleide scanning was good mainly for bones!! and perhaps the thyroid
