in hemopneumothorax, there is homogenous opacitiy at the inferior portion while there is homogenous luscency( no lung markings) superiorly. there will be blunting of the cardiophrenic angle and ( i forget the other angle). in diaphragmatic hernia ( usu occurs on left side), there may be blunting of the other angle i forget ( ie the one between the chest and the abd on the lateral side of the film) from some bleeding which accompanies traumatic hernias. the opacification is not usually homogenous cos it is from lung consolidation the luscency from teh air is also not homogenous cos it will show markings of loops of bowel. the markings may however not be obvious if the herniated structure is the stomach or the large bowel which may be dilated with gas.

multiple air fluid levels i think wud mean gut up there single is more likely hemopneumothorax

i remember the other angle, it is costophrenic angle. the air fluid level in hernia may also be from localised air/fluid in the dilated loop of bowel within the chest. again, it will not be homogenous cos we will see the markings of the loop of bowel. the cardiac border is likely to be delineated.
