A 65-year-old man with a history of severe chronic obstructive pulmonary disease is brought to the emergency department and admitted to the medical intensive care unit for respiratory failure. After intubation and optimization of ventilator settings, the patient is still hypercapnic. Which of the following will most improve this patient's hypercapnia? A. Decrease minute ventilation. B. Decrease tidal volume C. Increase carbohydrate intake and decrease fat intake. D. Increase fat intake and decrease carbohydrate intake. E. Increase the fraction of inhaled oxygen FIO2.

D. this q is about respiratiry quotient. firstly A and B are definitely wrong as they wud increase CO2 further E shud remind u of the important concept stressed by the physio dude...pO2 depends on CO 2 but this is not true vice versa that leaves us C and D. by choding D we have lower CO2 generation. lipids i think had a quotient of less than 1 ( that means that the amount of Co2 produced per O consumed is less..0.8 may b) whilst CHO have a quotient of 1

C. Though you are absolutely right about fats having a lower Resp. Quotient, you mixed up what that means!!! Resp. Quotient simply means O2/CO2 So a Lower Resp. Quotient means we have a lower O2/CO2 ratio...hence MORE CO2 produced for each O2 consumed. So more fats = more CO2.

The respiratory quotient (RQ) measures the ratio of the volume of carbon dioxide (Vc) produced by an organism to the volume of oxygen consumed (Vo). This is represented by the following equation: http://www.tiem.utk.edu/~gross/bioed/webmodules/r...

in fact this is what they're trying to find out here.. http://www.clinicaltrials.gov/ct/gui/show/NCT0025... Purpose The purpose of this study is to assess the impact of different feeding solutions on patients with breathing difficulty being supported by a breathing machine. The aim of the study is to determine if high fat-low carbohydrate feeding reduces the carbon dioxide production in patients with respiratory failure.

Study_ing you are right! My bad, I mixed them up. D it is. The metabolism of fat consumes more O2 relative to the production of CO2 than the metabolism of carbohydrates. (or simply put, for each O2 consumed, Fats produced less CO2 than carbs!) Thanks for the correction. Always appreciated =)
