it really sounds like sick sinus syndrome...i'd go for E...even if there was an ECG. sick sinus syndrome commonly presents with dizziness and lightheadedness especially in the elderly.

i'm only reviewing the NBME now and this Q shows the ECG..... my bad...shud be AV block answer A..haha...well thats only cos the ECG says so but really without the ECG it would have been E
