Hi idiopathic, thanks but please could you elaborate on the SLE aspect you have mentioned... is there an association with SLE and spherocytosis with Hb<5 ?

Yes. In autoimmune hemolytic anemia associated with SLE, you will of course have a positive direct Coombs but also you should notice spherocytes in the PB smear and the Hgb will be very low. If I am not mistaken this is a very common cause of death for SLE Px, but I am not 100% sure.

autoimmune hemolytic anemia is a potential complication in SLE but the commonest cause of death is due to renal disease( diffuse proliferative GN) and infections.

thanks idiopathic and asmi.... ...direct coombs is positive is all autoimmune anaemias....cold and warm.

then does quinidine and idoniazid ilicit cold or warm autoimmune hemolytic anemia? posts here seem to contradict each other?

Im sorry, my old post was wrong...quinidine, INH and penicillin all cause warm autoimmune hemolytic anemia...just remember the main cause of cold: lymphoma, mono, mycoplasma, and the rest are warm.

Its Warm in Georgia (IgG coats the antigen and is cleared by the spleen - coagulates on a slide at 37C) Its Cold in Michigan (IgM coats the antigen and settles in the vessels; cleared by complement - coagulates on a chilled slide at 4C)
