This question is not very clear. However, based on whatever is given it seems that this patient has renal injury (high creatnine, BUN/Cr ratio of ~10) and volume overload, I would go for immediate dialysis. The only answer which corresponds to this is creation and immediate use of arteriovenous fistula.

this was the exact que frm pretest. but the ans given is kaeyxalate enema for faster action. the expl as goes is: it takes approx. 2wks for the use of an AVfistula for dialysis, so if it has to be immediate dialysis it has to be a peritoneal dialysis. hence of the options given, kaeyxalate enema suits this pt.

Ya u need to wait for arterialization of the vein before being able to use the fistula.....
