18yr boy brought to to ER after a diving accident. He is awake and alert. has intact cranial nerves. able to move shoulders but can't mov e legs or arms. he is flaccid and has a sensory level at C5. appropriate Mx:

naloxone HCL

IV methyl prednisone

oral dexamethasone

phenytoin 10mg

hyperbaric oxygen

I would say IV methyl prednisone, given that he has signs and symptoms of acute spinal injury.

Yes IV steroids before anything else, for example before an MRI...

what helps in reducing damage from brain anoxia?

Guyz!!!!!!!!!!!!!! I have a doubt. cant this be a DECOMPRESSION SICKNESS after DIVING????? Then the answer would be hyperbaric oxygen to wash off Nitrogen dissolved in blood. What do you say. It may not be a spinal injury. Even if it is the treatment should be a SURGICAL DECOMPRESSION>

I was thinking exactly in the same lines msyamp..thought this was a case of bends..n i picked hyperbaric O2 ,but the expln given is diff...

well, says tht high dose IV steroids(methyl prednisone)given within 8hrs of spinalcord injury has some benefit

It takes about 2 minutes to prepare an ampulle of methylprednisone and give it, that would be my first step in management. As for the brain anoxia hypothermia.

He has the clear symptoms of spinal cord compression not of decompression sickness. The diving history is just a distractor in that direction.

I agree. Very localized symptoms. Even has a sensory level at C5. I think decompression would have more systemic events. Different sites of embolization, "the bends" (pain near joints), "the staggers" (vertigo and difficulty breathing).

Dexametasone 10-20 mg IV is the choice I found. I guess any IV steroid will help.
