This question is about cerebral toxoplasmosis, which answer is correct. Kaplans answer is: life long prophylaxis is necessary.

Thats wrong in my opinion. Harrisons 16th edition writes: secondary prophylaxis can be discontinued when initial therapy has been completed, they are asymptomatic and CD4 count is above 200 for 6 months.

So dont let Kaplan fool you with that question

not evdence base but here I go:

I did a rotarion of 3 months in infetious diseases department (AIDS), and they give toxo profilaxis for life to them in ths cases...


I would go for rest of life treatment.


I have harrisonŽs 16th edition, IŽll look for it tonight, so I can get what do they mean by saying seondary profilaxis.

Secondary prohpylaxis (Chronic maintenance therapy) means prophylaxis after true cerebral toxoplasmosis. Primary prophylaxis means prophylaxis without preceding cerebral toxo, it is instituted for patients with positive serology and CD 4 <100.

Its on page 1248 in Harrisons.

just curious are u guys reading from harrison for step 2 or just solving questions from its mcq book?

I read a lot in Harrisons during my internal medicine rotations and I am now preparing for Step2ck which I have scheduled in 2 weeks. At the moment I m using the Harrisons only as reference if I do not agree with answers or explanations or if I think I should repeat a topic in depth then I use Harrisons.

But wouldnt really use it for comprehensive step 2ck preparation. Just as reference if something isnt clear.
And i do not own the MCQ book, I have enough to do with finishing Kaplan Q Bank and UW Q Bank till my exam date
