A 35-year old man received a deep laceration with old rusted barbed wire while he was walking in the field. He was taken to local hospital. On examination there is 6cm(2.3in) long laceration on lateral aspect of leg, contaminated with dirt and soil. The laceration is bleeding profusely. He received 3 doses of tetanus immunization, when he was 23. What is the most appropriate next step in this patient?

A. Clean the wound only

B. Administer tetanus toxoid

C. Administer tetanus immune globulin

D. Administer tetanus toxoid and immune globulin

he has had 3 or more previous doses of tetanus toxoid. even though thatīs a serious wound, to administed tetanus toxoid would be enough. since tetanus immunoglobulin is used until the clinical diagnosis of tetanus is made, choice (d) is unlikely as well as (c), and choice (a) would be the answer if they add TETANUS TOXOID.

for me the answer is (b)
