yep long time cuz u been bizi ..its ok seems like my prep journal...weekend i just saw the forum cant study ..this weekend again some families friends comin just the week days ... tryin to finish surgery its 2nd read first i did when i watch the lectures..and doin UW questions .hmm no good% b/w 50 to 62 ...and i got the uw in books form too..before was makin the notes from uw now markin the points in books .im tryin to finish kaplan surgery by tommorrow..and there is 84 more questions ..also want to do q book and subjectwise test from kaplan..im not sure i can do that by tommorrow ..cuz m lazy...still can not make it to schedule my exam ..they still have some prob ..huh...may be becuz of that me lazyyyyyy ...gtg

well its been a crazy life...i know ive been having trouble keeping up  ...well i gotta run! but hey...im proud of u. seem to be getting stuff done and thinking urself lazy= sign of a great prestep2 student  ..is it ez to follow UW questions with the book..i know step 1 q bank wasnt hence me tot it wasnt worth buying the booK..good luck..looks like the step 1 and 2 forums will have to wait longer!

thanks..im at the barns n noble right now just started ...first of all i come n see here then start my studies today liberary gonna close at 5 but i need some more hours..thatswhy im here ..could,t make it yesterday.to finish ..but will try today..
for UW books..no these r just for like if u want to rewise the questions..n explainations..i think u need to do online..i think its like u r doin the block in exam though i dun go thru the exam but heard bout that and i feel like that..though im not gettin a good score ..but still i can learn the things im usin it like learnin tool..also conard fischers book is good..if u r in india or in pak u can get it easily..here in usa it must be expensive..gtg..see u n good luck

hey good luck..do check at the ecfmg website regularly ..sometimes they have a date availabe once someone postpones their old date. i really hope u get ur required date at ur given place. my prayers are with u.

one thing i really lacked in my step 1 owing totime constraint was revision. hence im gonna summarize some of the stuff as i go along...been a gap but nevertheless
- Depression is the most common association with Panic disorder .
- Pt on ACE inhibitors. BP- 140/80, pulse-64,RR-13. Muscle power decreased B/L and sluggish ankle reflex. Serum Creatinine Kinase is Increased. What is the most appropriate next step in management?
- TSH
- Hypothyroidism should allways be considered in patients with an unexplained elevation of serum CK concentration and Myopathy.
- Flail chest --> clinical findings disappear after positive pressure ventilation

Renal Vein Thrombosis most commonly (90% of cases ) occurs in age:

Testicular Torsion is best diagnosed by :
4.Radionuclide scanning
radionuclide scan of the testicles can be helpful to assess blood flow and to differentiate torsion from other conditions.
Scan is abnormal in torsion when it demonstrates decreased uptake in the affected testicle, suggesting no blood flow to that side.
- a sensitivity of 90-100% accuracy in detecting testicular blood flow.
- Color Doppler has a sensitivity of 86%, specificity of 100%, and accuracy of 97% in the diagnosis of testicular torsion when the presence of identifiable intratesticular flow is the sole criterion for diagnosis.

2 year old boy who has been previously well presents with foamy diarrohea, vague upper abdominal discomfort and failure to thrive for 2 months. weight is on the 25 th percentile, stool test reveals steatorrhoea with fatty acid crystals ,the most likely diagnosis is :
A. giardia lamblia
B. coeliac disease
C. cystic fibrosis
coeliac dis. is possible, but would usually have an earlier onset when glten is introduced into the diet from about 6 months.
CF with significant malabsorption would present in early infancy
giardia lamblia is very common in young children, particularly very common in day-care in US. day care also hints.

A 28 year old woman presents with lethargy, dizzy spells and occasional syncope. Dusring a dizzy spell the heart rate is 32 beats/ min. and the BP is 85/60 mmHg.
Your diagnosis and / or explanation:

C. Structural disease of the cardiac conducting system (Sick sinus syndrome)
D. Sinus tachycardia with 4:1 atrioventricular block
E. Iron deficiency
- C. is not the answer
Structural disease of the cardiac conducting system- the sick sinus syndrome could produce episodic heart block and dizziness but would be very uncommon in a woman of this age group-28 years
- Addison diesease may be associated with mild bradycardia, but not a pulse rate of 32 bpm, a rate which strongly suggests a significant degree of heart block.

- Iron deficiency that produces anemia will tend to produce a mild tachycardia rather then a supraventricular tacharrythmia with block. I nsulinomas can present with hypoglycemia and syncope but are not associated with such a low pulse rate
- Sinus tachycardia with a 4:1 atrioventricular block would be the MOST likely cause

elderly, limb girdle aches and pains.
esr helps.
DO NOT USE HIGH DOSE STEROID because the pt may swell up three weeks later and you don't know what you are treating with !
The trick is to give the pt a very low dose steroid for 3-5 days and see if she responds.
All pts with PMR should respond in 3-5 days of prednisone (low-dose) !
....if we consider PMR we can find subacromial and subdeltoid bursitis and glenohumeral joint synovitis in MRI of the shoulder and also MRI of the hands and feet demonstrates inflammation of the tendon sheaths in many patients but X-ray findings of osteopenia as it is cited in the text books are very rare

parotid swelling
pt. has acute surgical parotitis ( think s aureus)-sialadenitis , usually seen P/O in elderly pt.swho have poor dental hygine and who have been intubated, very frequent complication in US.
