- postpone trip inform pt of greater risk to pregnant ladies i f Have to, non pharma measures ( eg prevention of mosquito bites) benefits of pharmacotherapy outweigh those of malaria in pregnancy prescribe either chloroquine or mefloquine based on resistence patterns Experience with chloroquine and limited experience with mefloquine indicates that they are safe to take during pregnancy, including the first trimester
AVOID
atovaquone/proguanil,
doxycycline,
or primaquine.

also yesterdya i had planned to sumarize that thread but was too lazy..
areas with malaria risk
- Africa, South America, the Indian Subcontinent, Tajikistan, Asia, and the South Pacific
integrating malaria resistence
- If you are going to Mexico, Haiti, the Dominican Republic, and certain countries in Central America, the Middle East, and Eastern Europe --> take chloroquine
- If you are going to a chloroquine resistant area such as the old world (africa, India, south east asia), mefloquine is fair. (source frank 100 for this one..rest cdc)

Third Heart Sound S3
Results from increased atrial pressure leading to increased flow rates, as seen in congestive heart failure, which is the most common cause of a S3. Associated dilated cardiomyopathy with dilated ventricles also contribute to the sound.
Less commonly, valvular regurgitation and left to right shunts may also result in a S3 due to increased flow.
May be normal physiological finding in patients less than age 40.

Fourth Heart Sound S4
Clinical significance:
Seen in patients with stiffened left ventricles, resulting from conditions such as hypertension, aortic stenosis, ischemic or hypertrophic cardiomyopathy.
In patient with mitral regurgitation, suggestive of acute onset of regurgitation due to the rupture of the chorda tendinae that anchor the Valvular leaflets.

hey study-ing, you are a real whiz kid!!! how do you manage to pack in so much studying in a day? i am still struggling with kaplan surgery. i am done with the first part and am about to start the vignette. i really admire you.
i am also sitting the exam in oct. i think 2 months should be adequate going by step 1 experience. it is better to read like you will die ( you never die ) than to take your time. i forgot so much of what i knew by dday cos i had read them so long ago.

im also in same boat..as u r. i been worried about it may be i did wrong decesion but its ok i did it now i have to prove it its not wrong decesion...yes studyin is right ..u can find the royal george,s mail in exam experience it will help u too..
for me i only did these subjects from kaplan book ..if i can remember it enough for me

well these days im more into my books( usually too bored) .. am done with gyne videos ...am doing GI now....havnt got usmle world yet. perhaps by the 25th.
im sorry i have no idea about C F's q book but he is a kaplan teacher right so i guess he aint bad...its the first time i heard of this book..will tell u if i come across anything.

q book has got 850 qs of which ive just done two testss...havnt touched usmle ez in ages but it was a good starter..( if ur the sort who gets so sick of stuff sometimes u keep staring at it, then u may try it but its definitely not on my MUST do list.
