Best initial test for gout- arthrocentesis. Most accurate test-synovial bx.

Acetaminophen overdose after 3 hours

1. fno lavage as its too late

2. look at dose ingested, if >7.5 in adult, give antidote ( n-acetylcysteine)

3.if less, do acetaminophen levels. compare with nomogram and decide treatment

infants born of mothers with diabetes mellitus have higher risk of cong malformations incl cong short colon!!! ( may be responsible for meconium plug)

Crohn's dz

  • Mild to moderate disease The patient is ambulatory and able to take oral alimentation. There is no dehydration, high fever, abdominal tenderness, painful mass, obstruction, or weight loss of more than 10 percent.


  • Moderate to severe disease Either the patient has failed treatment for mild to moderate disease OR has more pronounced symptoms including fever, significant weight loss, abdominal pain or tenderness, intermittent nausea and vomiting, or significant anemia.


  • Severe fulminant disease Either the patient has persistent symptoms despite outpatient steroid therapy OR has high fever, persistent vomiting, evidence of intestinal obstruction, rebound tenderness, cachexia, or evidence of an abscess. Remission The patient is asymptomatic OR without inflammatory sequelae, including patients responding to acute medical intervention.

For Idiopathic Pulmonary Fibrosis.

best initial test-CXR. next test CT looking for Honeycomb lung or ground glass.

Most accurate test is Biopsy.

[left]Initial Treatment

• Inhaled rapid-acting 2-agonist, usually by nebulization, one dose every 20 minutes for 1 hour

• Oxygen to achieve O2 saturation ≥ 90% (95% children)

• Systemic glucocorticosteroids if no immediate response, or if patient recently took oral[/left] [left]glucocorticosteroids, or if episode is severe

• Sedation is contraindicated in the treatment of attacks[/left]
