1. the presence of both increased irritability and AV nodal block (such as paroxysmal atrial tachycardia with a 2:1 AV nodal block). is suggestive of..

digoxin toxicity though With toxicity, practically any arrhythmia can be seen.

2. ST segment changes are pretty characteristic, with their "reverse tick" conformation. These changes are not indicative of toxicity, but merely the presence of digitalis

4. how to manage it?

a percutaneous renal biopsy ( to grade it and to follow response)

treatment modalities include

high-dose steroids alone,

high-dose steroids plus antilymphocyte globulin

or high-dose steroids plus OKT3

(the latter was murine monoclonal antibody to the human CD3 complex).

hence ON PHYSICAL EXAM IF U FEEL PULSATION MEDIAL TO UR FINGER ITS AN INDIRECT HERNIA

  • in giving a dose of antivenom the age or weight of the patient DOES NOT MATTER. all that matters is the amt of venom injected.

i think studying from the clinicl vignettes first and then doing the initial summary chapters is a better idea than trying to learn the inital ones first...

well i neeed to have some sort of plan..is just scary coz i feel i cant stick to it.

im silent user but cant be silent no more cuz i cant stick to my plans too...and only 2 months to go ...may be i,ll join ur prep journal to share my frustrations ...want to do this much cant do that much anyway still have to do n go tryin tryin tryin
today i went to prometric cuz they have some prob in system cant schedule the test...so i went to centre and ruin half day ...........im back now system is down ..so will see tommorrow if i can schdule the test ..look like im jumping from one topic to other ...hmmmm cuz im studyin psychiatry..lol ..only 6 hours to study today
so i need to finish psychiatry from kaplan 3 chapters and have to finish remaing 56 questions with explaination from uw then have to finish biostatics
i hope i can do it

of course not ....its a relief ..in the mega organized budding doctors community, i feel an outcast!! following a timetable is next to impossible..ppl dont realize its as much of god's gift as IQ...

hey i dont mind biostats so please feel free to discuss ur difficulties

3 most common porphyrias..

abdominal pain...think Acute intermitent porphyrias

Photosensitivity

acute---think erythopoietic porphyria

chronic --PORPHYRIA CUTANEA TARDA
hmm difficult to retain



Abdominal Pain= Acute Porphyria ( intermittent)

finished psychiatry 3 chapters n did the 27 qustions uw ...dang cant remeber the names of drugs disapproval all mixing in my mind is there ant formula to put them in my lil brain
all the questions i did wrong was the drugs drugs drugs confused im doin all the questions in time mode and never short of time but wrong answers sad cumultive is 60%
hey studyin can u suggest me either i should do q book or not i got q bank cd too but too scared to do all those things ..
i,ll start biostatics after puttin these all medicines names in my brain i think take me long

i am doing q book these days..i havnt heard ppl praise q bank that often..in fact ppl do their step 2 and step 3 q boks before step 2..

relax sheeezo. psych meds are not that tough if u dont allow them to be..in fact u shud study them with the aim of generalization...back then( in med college) i put in a lot of effort in understanding what the syptoms are eg of neuroleptics.not just learning their names but recognizing what they r...its tough work initially but definitely worth it

  • to distinguish a SAH from a putamenal hemorrhage look for the neuro deficits.
      contralateral conjugate paralysis--> putamen nerve deficits ( CN II--PCA aneurysms, Increased icp--CNVI false localizing sign)
  • signs of high ICP ( papilledema, headache, n , vomiting etc ) are common to both

approach to vignette with parkinsonism
  • if SYMMETRICAL vertical gaze limitation and axial rigidity think progresive supranuclear palsy
      MIDBRAIN AND BG involved
    others ( dementia involves frontal lobe more hippocampal/memory systems) initial symptom-falls Modest response with levodopa imaging unremarkable


  • if asymmetrical vertical eye movemnts ok falls late into disease pronounced improvement with med think parkinson's disease



Etcs


cause of Bilateral pallidal necrosis--> Co poisoning

gaze unafected

vascular parinsonism is a d/dt but requires heavy burden of disease

when freaking out

look for VERTICAL gaze in case

Age - Risk of malignancy increases with age
  • Risk of 3% at age 35-39 years
  • Risk of greater than 50% in patients older than 60 years
  • Usually, nodules are identifiable by the time they are 8-10 mm on chest radiographs
  • size alone is a pior predictor in a give individual

  • The growth rate refers to the doubling time of a nodule, ie, doubling in volume.
  • volume of a sphere equals 4/3 pr3; therefore, a 26% increase in diameter on a chest radiograph represents one doubling in volume. ( 2d on x-ray, not 3 d)
  • For example, an increase from 1-1.3 cm equals one doubling. A 1-2 cm increase relates to an 8-fold increase in volume.
  • Bronchogenic carcinomas usually have a doubling time of 20-400 days
  • Doubling times shorter than 20-30 days are seen in infections, infarction, lymphoma, or fast-growing metastases.
  • Doubling times greater than 400 days are typically benign
  • Absence of change in size of a nodule over 2 years is highly suggestive of a benign lesion.
  • of course exceptions occur. CXR may not accurately dectecta 3mm change
