SAAG is serum ascites albumin gradient as the name suggest is the gradient(differenc between serum and ascitic albumin), its significant is that it help in differentiating the cause of ascites if it is >1.1g/dl it points towards 1. hepatic congestion.(CHF, costrictive pericarditis, tricuspid insufficiency,budd-chiari and venooclusive 2. Liver disease(cirrhosis etc) 3. portal vein occlusion. <1.1 then if peritoneum is normal then 1.Hypoalbuminemia(nephrotic syndrome and others) 2.chylous ascites 3.nephrogenic ascites. 4etc if peritoneum is abnormal then 1. infections( bacterial peritonitis,T.B peritonitis etc) 2. Malignanat conditions(peritoneal carcinomatosis, mesothelioma,HCC etc)
