A 30 y old women comes to his primary care physicians with complaints of burning on urination. Urinanalysis reveals Enterobacter.

What is important to know about the drug of choice?

A) It is a strongly associated with congenital anomalies and should therefore be avoided in pregnancy

B) The drug is severely ototoxic

C) It inhibits the peptidyltransferase-activity of the 50S-ribosomal subunit

D) It has to be administered with cilastatin to prevent renal breakdown

E) It is associated with the “Red man syndrome”

A could be many drugs!
B is Aminoglycs
C is Chloramphenicol
D is Imipinem
E is Vanco

DOC for Enterobacter is Imipinem.

I'm curious - why would you pick a big gun like Imipenem on an Enterobacter? E. aerogenes is pretty sensitive. I will admit that some strains of E. cloacae are multiresistant. We suppress results for Imipenem unless infectious disease specifically requests them or in certain circumstances, i.e. ESBL's or multiresistant organisms.

Ive would have guessed Imipenan being only about 70% sure.///I had a similar comment from bactitech about 2 months ago. As I remeber I call it one of my "SPACE" {not easily treated} organisms Serratia, Psuedomonas, Actinobacter, Citrobacter, and Enterobater. Bactitech can you comment on treating any of these?

Yes, D is correct.
Imipenem is the drug of choice for Enterobacter (even if you can also use cephalosporins. But these are not in the answers and are also only 2nd line)

C is not only cloramphenicol, but also macrolides, lincosamides, clyndamycin
