A 52-year-old woman presents to her physician for a check-up. She is recovering from a wrist fracture after a fall. Dual energy x-ray absorptiometry of the hip had shown her to have osteoporosis. She became menopausal at age 50 and did not begin hormone replacement therapy because of a strong family history of breast cancer. She now fears a future hip fracture and would like to begin a bone loss prevention regime.Which of the following pharmaceutical agents is most appropriate for this patient?

A. Calcitonin nasal spray
B. Oral conjugated estrogen
C. Raloxifene
D. Tamoxifen
E. Transdermal estradiol

Id tell her# 1)1200 mg Ca++& trace magnesium #2) 1000 I.U. Vitamin D #3)weight bearing exercise (not swimming) and bone stabilizer like (Fosamax) alendronate. As far as I know Tamoxifene is an antiestrogen used after estrogen sensitive breast Ca surgery

addendum-it was only as of April 2006 that raloxifene was found to be as good as tamoxifene in decreasing breast ca/we know of the osteoporosis part.. How do you like these big time side effects- chest pain, trouble breathing, thromboembolic events, that enough? How about stroke and cataracts also??

**As far asI know it must be "ESTROGEN SENSITIVE" breast ca for raloxifene to be effective.

Why not Calcitonin nasal spray? I found that it used in corticosteroid-induced ostoeporosis, but also it is recommended for postmenopausal osteoporosis. I think the clue is that raloxifene will help to prevent breast cancer. When then Calcitonin nasal spray will be used?
