in valslva the venous return to left heart is decresed and in handgrip and squating the venous return to left heart is increased so in simple valvular lesion in the valsulva the mumur of left herat shud get soft and by handgrip and squating they shud get loud but in hypertrophic cardio myopathy as the lesion is dynamic increses the return will cause the obstruction to worsen and very little blood flows out of left heart so murmur softens and in vasulva as the venus return decreses so the dynamic obstruction will not be that much worse so we hear parasoxically a loud murmur

stydying this the best i cvan produce t this time

Ok.. here is valsalva:

Valsalva... positive Intrathoracic pressure, dec. VR of blood to Right heart, decreases cardiac output... increases HR...

Effect on murmurs
increases intensity of HOCM (IHSS)
moves systolic click murmur closer to S1 in MVP
decreases intensity of ejection murmur of aortic stenosis

Hand grip and squatting increase VR, so does passive leg lift when supine.

i think handgrip promotes the venous return rom the deepp veins to herat due to contarction of muscles
