A RECENTLY VACCINATED 18 yr old PThad lymphadenitis

had her LN biopsy taken which showed

B9 paracortical expansion and scattered multinucleated giant cells with eosinophillic cytoplasmic and nuclear inclusion bodies...

which of the following VACCINES were most probably responsible for her lymphadenitis?

Hepatitis B

Measles

i was confused btw these 2 choices. the paracortical expansion points towards a viral vaccine. however because there were nuclear inclusion bodies, i chose hepatitis B. the correct answer is Measles however. in the explanation they say the cells described are fused lymphocytes called Warthin Finkeldy giant cells...my confusion is how measles, an RNA virus is gonna cause intranuclear inclusion bodies...also duz hep B have cytoplasmic ones??

i dunno. i just know the answer is measles. does hepatitis vaccination give lymphadenopathy?

The HepB vaccine is only a capsule conjugate vaccine. It's too dangerous to give a live/killed HepB vaccine, cuz or risk of Hepatocellular Cancer. So there is ZERO chance of any infection with a HepB vaccine.

The Measles vaccine on the other hand is a live virus vaccine.

I don't think anyone really expects us to know about "Warthin Finkeldy giant cells", so to answer this question all you really had to know are the basic concepts of the different vaccines.
