Kaplan Qbank USMLE

Hi, all

I have read Kaplan Physi that states T4 is most imoprtant with down regulating to TSH instead T3 although T3 is the active form. A q from qbank see attached file. basically it means T3 is down regulating TSH and T4 decrease. It is really confused. So which one is regulate TSH? T3 or T4?Please help. thanks!

this concept has cozed cnfusion before too

what is MORE importaant for feedback is T4. however, when pts are given T3 exogenoisly, TSH levels do fall. remeber T4 is converted to T3 in the tissues . perhaps more T3 means conversion of t3-->t4 is less and hence more is available for feedback

yes study-ing. there is usualy more t4 circ than t3 but t3 is the biol active one. t4 is converted to t3 in the throtrophs and this acts as negative inhibitor for tsh release adn subsequent t4 prod. so if pt is given t3, level of tsh and t4 will drop.
