yes, you can touch the patients, do P/E, make clinical histories. yes, all this in an externship.
In an observership you can also do this but it depends on the attending you have. they can also let you to observe as the name says, it varies.

I did observership in a private clinic but the attending was also a internal medicine professor in a medical college. So he let me do the history and physical exam everything.
I will be doing another observership in a major hospital. Will post about it later if this thread continues.
