2. concentration

#3 tell patient 3 or 4 numbers or a statement to remember then ask him to repeat them.

not sure but here is what i think.
attention: show flash cards, cover and ask what they see
concentration: ask to deduct 3 from 100 sequentially eg 97,94,91
