In this case which one would you choose as the main microorganism, s.aureus or klebsiella pneumonie? case: 65 year old woman in nursery with flu like symptoms which develops fever, cough...(pneumonia) and mental status changes. CXR: cavitary lesion. ok I think it should be klebsiella ( old ,community acquired, abcess) but staph causes abcess and is common in elderly too! So what do you think?

I'd pick Klebsiella. Don't forget a common thing in Klebsiella is also that it loves to reside in the fissures of the lungs!!!

Its Klebsiella produces pneumonia in elderly patient especially in nursing homes.. Staf aureus produces pneumonia often superimposed on influenza pneumnia , also in cystic fibrosis patients, produces pleural effusion tension Pneumotocysts danger of pneumothorax.

flu like syptoms in the question cant it poitn to influenza infection?

yes, muzammil, that's what I doubt too, and what about mental status? ....the problem is that the answer given is s.aureus...so I'm confused. and you know staph causes pneumonia in elderly too.
