54 yo man, who was shooted in the abdomen and goes for extensive abd surgery, he receives 20 (twenty) blood units in the operating room, 2 days later after a succesfully surgery, he develops an increment in his Creatinine to 3 and in the Urine you see casts, no fever
what is the cause of his creatinine increment
(It was something like that the question in my step 2)

what kind of casts?

what were his vitals?

:idea: I would think about prerenal azotemia. He probably lost a lot of blood and his kidneys were some time without receiving enough flow to make those glomeruli work. I would say that volume status derangements would be the first thing to think about in a post op renal insuficiency.

:idea: If the lack of flow was long enough, he probably underwent acute tubular necrosis, which would also explain the casts.

Of course thereīs a long list of differential diagnosis for high creatinine.

Were there any options?

I cannot remember the options but I choose your answer too, my cue was the casts too
