Did you noticed fibrosis in the Q stem.. its cirrhosis and so the portal pressure will be high

well the question asks what wud cause the histology to develop in such a way. portal HTN is a result of, not a cause of it.

btw our dear professor has stressed on this point...(the real one, that is)

collapse of architecture causes the BM to fall too..thats what I know and then there is increased portal pressure...

because HEPATIC VENOUS PRESSURE is almost the same as in Inferior vena cava...as they drain into it.... smiling face

i am so sorry. i cam e across the question again and realised i was making the exact mistake i h was warning ppl about!!

the question asks what the result of the distortion is. so its elevated portal pressure for sure

if the question asks about consequence of distorted architecture, answer would be increased portal pressure but if it asks about what causes the distortion then increase in CVP will be right ( from right heart failure----nutmeg liver, cardiac cirhosis)

liver's "zone 1" (around the hepatic vein) is most sensitive to damage (remember goljan acetaminophen toxicity also rhf&nutmeg liver) so i think it is due to incrased cvp, it is either c or d.
(also i recall goljan ruling out portal vein because liver has dual blood supply, if portal vein is screwed hepatic artery will do just fine. )
in actual exam i'd choose hepatic vein.
