Estrogen stimulates cortisol-binding protein, CBP synthesis. In pregnancy estrogen goes up, more CBP, more cortisol bound to CBP, less free cortisol, more ACTH released in response, free cortisol level restored with CBP bound cortisol still being elevated.

So if any young female( female of reproductive age group) if she has increased plasma ACTH, Increased CBG, increased Total cortisol but normal free cortisol.............always points to pregnancy or OC pills.
