If I palpate the abdomen and the SP acts as if there is RUQ pain, and I want to check the liver span or check Murphy's sign. This might be painful to the SP - should I or Shouldn't I do these ? Is it considered repeating painful maneuverts ?

write U/S abd. in work up and tender to light touch in PE. If will fail that particular case if by chance you illicited painful manuver twice after subject told you.

Is this mean we can not check "deep palpation" or "rebound tenderness" if Patient already has pain on superficial palpation? Not sure.

that sounds tricky. if the patient has exquisite pain on even 'touching the skin' then i guess we can forego deep palpation and rebound......and not forget to mention that in PN...O/E not able to elicit those manuevers due to extreme pain.
