can someone please help in differentiating the types of hypersensitivity reactions they ahve? am particularyl confused about type 1. i know it has type 4 which is cytotoxic T cell mediated but some types of type 1 have type 2 reactions too. correct?

type 1 DM is of two types

A)90 % is immune mediated. ( Islet cell antibodies are PRESENT, HLA association positive))

B)10 % is idiopathic

it has a high HLA association. Family history is less important however.

HLA DR3 and DR4 -->susceptibility

85 % of patients have Abs against islet cell Ags

Type 2 has a strong genetic influence. Family history imp, HLA association is NOT.

islet cell antibodies not present here

however, on pg 424, javetz micro, 7th edition, insulin dependent DM is listed as a disease which involves a significant cell-mediate and antibody -mediated response

on page 427,

IDDM or type 1

the major damage is T cell mediated ( though antibodies against the various Ags also are produced.

autoreactive T cells destroy the islet cells of the pancreas

the main against which the T cell attack is directed is the islet cell Enzyme GAD.



in insulin RESISTENT DM, antibodies aginst insulin Receptors may be found.

moral of the story

its diffficult to understand!

lets see..i think ikts because there is a lot of subtypes and thus overlap..

Type 1...antibodies are produced in the majority. however the major mediator of damage is the T cell. HLA association high, family history less inportant however

in type 2, a variety of subtypes exist

..in INSULIN RESISTENCE , antibodies to insulin rc occur so its a type ii hypersensitivity similar to Myasthenia

overall, family history is imp, amyloid is present, HLA not imp

( thanx for pointing it out tolito...its scary i didnt know it so close to my exam )

thanks so much study-ing. the closer we get to the exam, the more we notice stuff we took for granted and the better we are at deciphering their missiles at the exams in order to produce the appropriate antimissiles.
