Mz XYZ, AFter doing your examination and asking your history I have come to alist of possible conditions u may be suffering from. It could be anemia which is a lack of blood in your body, hypothyroidism that is underfunctioning of your thyroid gland or even a chance of carcinoma exists ,given your history ( i am assuming there will be some history or risk factors here ) but we have to rule these out with a few tests so we can come to a conclusion. I will keep you posted as soon as we get the results and we can take it from there. Will u be willing to give us some blood samples for the required tests? Also Mz XYZ, i would like o get an x ray of your chest and a CTscan of your abdomen. I would like to do a rectal exam but we will do that next time.

Do u have any questions can answer before I go ?

Some one pelase tell me how is that ????

I wouldn't mention cancer but would mention internal bleeding, hypothyroidism, CRI, etc. Cancer is a big word! Even though there might be a chance of this do to history which there is a good chance there will be a case of patient who smokes a lot or familial polyposis or Chron's disease, etc. Do mention all the test that will be done. Mention for what if asked. You can say that do to history(smoking, bad eating habits, family history) you want to check lungs or GI to measure everything is OK. Colonoscopy, x-ray, CBC, BUN, T3 & T4, etc.
Anyone else has other opinions?

if the Sp is elderly I think that we should mention that we shall be doing stool FOBT to rule out any chance of CA COlon as it is also recomended screening above 50.
We would have to take the risk of the Sp reacting unfavourably to 'cancer' in this case. What do u think?

How about counseling the fatigue case to do sm exercise as it will improve energy levels e.t.c.....
