In general after history and physical we need to tell patient what are few possiblities (DD) and work up plan. In the peds case, since you have not seen the child, do we still have to tell DD based on history or just say that you need to see the child which is very importand after that you will discuss possibilities. Any suggestion, I am taking exam very soon.

You say Mrs/Ms. XYZ, Thank you verymuch for giving all details of the (C.C.) that your child( name) is facing,but to come to an exact diagnosis, I need to do a Phy Exam on your child( describe area acc. to complaint ) after which Some tests like blood /urine tests may be required to be done. Can you bring the child to the hospital? I will personally examine him and then we can discuss what next steps we have to take to treat him.I am really concerned about his well being. Is there any difficulty for you to bring him here?

Adult Summarise H/O and P.E. and describe what D> D. You are considering also workup plan- Inv to come to a final diagnosis. Next you councel -- DM/ HT - diet, lifestyle, exercise. Smoking -- Alcohol -- ( depending on amount ) Safe Sex practice -- if H/O Multiple / unprotected sex./STD case.- then describe that both u and partner should undergo tests and treatment.Also it will be better if you both undergo tests for HIV screening,which will require your consent. Old Person not seen doctor for years.- Advice to also visit Pri care physician and advice about colonoscopy / prostate ca screening- male and colonoscopy / breast screening -- female. Dom Violence -- Support groups.I can give you phone numbers or you can make calls from my office if you feel that you cant do it from home.Is it OK?
