A 24 year old woman is brought to the ER because of a 3 day history of migratory polyarthritis. Over the past 24 hours, she has become unable to walk because of a tenosynovitis of the right patella. Exam of fluid from the right knee shows 50,000 leukocytes/mm3. A gram stain of this fluid and a cervical exudates show gram negative diplococci. Analysis of her serum for complement activity indicates that a total hemolytic complement activity is only 5% of normal.
