Thanks a LOT robin. People, please answer my question: how frequently are the NBME exam questions changed? If I take the exam in say, Dec I hope these won't be repeated. I believe the answers are your own...or are they available somewhere, on their website perhaps? Thanks. Thanks a LOT to robin, again.

These are my own assessments of the correct answers, feel free to discuss. And before I say anything, thanks to robin for giving all this info, plus the answers. Hopefully we can all discuss these, and come to the right conclusions. This is not an exhaustive list, I need to go through these Q's again. Block 1 18) C one minus beta equals power of the study. Look also at the p=.06, very high, you need more test subjects. There are very few subjects to come to any real conclusive data. Also, I believe the study population is appropriate (old ladies, with osteoporosis, sure). 22) B the spinothalamic tract (and all other tracts, minus the uncus) have secondary neurons that pass through the thalamus. C,D,E are all pointing to the internal capsule. 29) E? this is obviously duchenes MD. Immediately when I saw the word “replacement” I went for this answer, however, I always thought of it as replacement with adipose tissue. That’s why you get the pseudohypertrophy of the calf muscles. It could be connective tissue stroma. But there is definitely a replacement of muscle tissue, and hence the huge CK levels. 32) E there is a very similar q-bank question. It produces all these symptoms, the thoracic duct enters at the jugular subclavius angle, and the lymphatic blockage anatomically would produce all these sx. 46) C for sure it’s hypomagnesemia. You can read about it here. http://www.emedicine.com/emerg/topic274.htm Block 2 1) B I thought this made sense. 26) C Though the time frame suggests this has been going on for 6+ months, I think schizophrenia is a distractor. Given his age, and his description of what’s going on, sounds like he’s taking some LSD. 28) E? But I’m not sure. 44) E Ill defined borders, pretty sure this is right. Block 3 20) A It’s Burkitts lymphoma. Look at the starry-sky appearance. Don’t be tricked, those are not Reed-Sternberg cells that you’d see in Hodgkins lymphoma. Those are large, binucleate cells (owl’s eye appearance). Also remember the c-myc oncogene, the translocation t(8;14), and the association with Ebstein-Barr Virus (mono). 29) C I didn’t see RA in this one, but I might be wrong. Looks more like polymyositis. Block 4 44) B The reason why: The 5-alpha reductase is responsible for making DHT, which works on external structures. The external genitalia would appear as a clitoris and vagina, however, this person would still have the internal structures (the mesonephric and Wolffian differenciated structures). Testosterone is responsible for these (along with Mullerian inhibitory factor in utero).

all good points... i'm still saying that Block 3, #20 is still burkitts lymphoma. i've reviewed the slides carefully, and those are not reed-sternburg cells. the ones in this question are multinucleated, not binucleated. i think they want to throw you off by giving you the high power and low power microscopy (the low power shows the starry-sky), because the high power kinda looks like hodgkins, but there is a difference. reed-sternberg cells have a very characteristic appearance.
