When the question reads "What is the most appropiate management at this time?" --- are they asking for the inital test or the most diagnostic test. Actually, there is a GOLDEN RULE of management for most specific disease, however, there are mnay other answers as second choices. What the question asks is the most appropriate answer ? It is very simple to understand. What is the most appropriate anti-hypertensive medication for a diabetics? You can use CCB, ACEI, alpha-blocker, anything but the most appropriate based on "clinical evidence based result" is ACEI !
