I was wondering if anyone can help with my problem with not knowing what exactly the question is asking.

When the question reads "What is the most appropiate management at this time?" --- are they asking for the inital test or the most diagnostic test.

and is there a difference when they ask for the most appropiate manangement and the next step in management.

your clue is "at this time", that means that it depends on the case presentation. You can be at the begining, middle or end of the precess of diagnosis or treatment. So that will vary in each case.

When they ask "what is the next best step in ......" is the same as " what is the most appropriate ....at this time..." BUT when it reads ONLY "most appropriate" and it doesn´t specify "at this time" it means overall.

Actually, there is a GOLDEN RULE of management for most specific disease, however, there are mnay other answers as second choices.

What the question asks is the most appropriate answer ?

It is very simple to understand.

What is the most appropriate anti-hypertensive medication for a diabetics?

You can use CCB, ACEI, alpha-blocker, anything but the most appropriate based on "clinical evidence based result" is ACEI !
