From: www.usmle.org/step2/Step2CS/Step2Indexes/Step2CSIndex.htm The USMLE Step 2 Committee is responsible for setting and monitoring pass/fail standards for the Clinical Knowledge (CK) and the Clinical Skills (CS) examinations. Initial standards for the CS examination were set by the Committee in November 2004. Because this process was completed with less than a full years cohort of examinees, the Committee decided to closely monitor cumulative data on CS during the following year and to undertake a review of the CS standards in the Spring of 2006. This review has been completed. Based upon the review, the Step 2 Committee has decided to raise the performance levels required to receive a passing outcome on two of the three Step 2 CS subcomponents: Integrated Clinical Encounter (ICE) and Communication and Interpersonal Skills (CIS). There will be no change to minimum passing requirements for the third subcomponent of Step 2 CS, Spoken English Proficiency (SEP). The new passing requirements will be applied to Step 2 CS examinations taken on and after July 16, 2006

That's very bad news - I'm going to take CS after July 16, 2006 - this means that for me this exam will be more difficult than for the previous examinees and the 'standard' 1 month study for CS with the 'standard' resources may no more be enough to pass.

Not good news certainly!! I just wish now that i hadnt procrastinated so much! Do u suppose this means there will be changes in the exam pattern? maybe like the number and kind of questions asked by SPs.....i wonder why USMLE doesnt post this bit of additional vital info..! BTW, What exactly does Integrated Clinical Encounter entail? Hx and PE?

You're right drk.It should be specified what is exactly meant by "raising the performance levels".Although I was expecting this I am still quite taken aback cos I scheduled my CS a little over two weeks after CK...*gulp* Oh well...hopefully the bar hasn't been raised too drastically. Heres a table from the USMLE bulletin for the CS subcomponents. Table 3: The Subcomponents of Step 2 CS Integrated Clinical Encounter (ICE) Subcomponent - Data gathering – patient information collected by history taking and physical examination Documentation – completion of a patient note summarizing the findings of the patient encounter, diagnostic impression, and initial patient work-up
Communication and Interpersonal Skills (CIS) Subcomponent - Questioning skills (e.g., use of open-ended questions, transitional statements, not interrupting the patient) Information-sharing skills (e.g., avoidance of jargon, responsiveness to patient
questions or concerns, provision of counseling when appropriate) Professional manner and rapport (e.g., concern for patient's comfort and modesty, examinee's attention to personal hygiene, expression of interest in the impact of the illness) Spoken English Proficiency (SEP) Subcomponent - Clarity of spoken English communication within the context of the doctor-patient
encounter (e.g., pronunciation, word choice, and minimizing the need to repeat questions or statements)

This is news is disturbing as I am booked for 19th July, but I feel that if we are prepared well and do the right things, it should not affect us. May be this rule is applicable for attempts when candidates made some mistakes in most counters and still passed and now only thos will pass who make some mistakes in few counters. It is just a hypothesis. I feel that we will have to prepare better and hope for best. Nothing can be done about it by worrying. Good luck to all. Lets see what happens in the first few days of the new change.

I don't think anything will be different as exam experience, I think what changes is the percentage of things to do on the SP checklist and the things to write on PN in order to pass (you'll need to do more to pass than before). if anything was different in the patient encounter they would have said so. just do your best and make them mark as much as possible on that checklist!

docswapna, from another us resident.. integrated encounter is fine; just one or two more clicks, but communnication and interperonsal skill may be a bit harder since it is subjective..
