A sexually active 18 year old woman comes to the physician's office because of fever,lower abdominal pain and a purulent vaginal discharge for the past 3 weeks
A tender mass is palpated in the right lower quadrant on bimanual examination

Dx.-PID "pelvic inflamatory disease"

Right lower quadrant.-ovary

Now if there was BLOOD I agree fallopian tube{ectopic pregnacy}

The patient had purulent vaginal discharge... so I agree with option B... its PID that could have collected an abscess on the fallopian tube, consistent with all the characteristic given smiling face

Very typical of salpingitis....I never heard of ovaritis though! .....so the answer according to me is Bsmiling face
