I think D is the only possible correct answer.

This case doesn't match the DSM IV criteria for bipolar I disorder: there is no mania.

Dementia is not possibile, because the onset is too rapid: you can't get dementia in a week!

Schizophrenia is not correct, because this diagnosis requires at least 6 months of positive and negative symptoms. Furthermore, in schizophrenia, there is no impairment of consciousness: the patient is awake and oriented in person, place and time.

Malingering is not suggested by the question stem: what should be the patient's gain, in this case?

So, delirium is the correct answer. A key criterion to diagnose delirium is an impairment of consciousness level ("he does not stay awake"). Often, delirium is associated with a medical illness (in this case, there is "incontinence").
