A 45-year-old man develops a fever 4 days after an abdominal operation. Blood cultures grow Staphylococcus aureus. Which of the following is the most likely site of infection from which the organism gained access to the blood stream?
A ) Anterior nares
B ) Sputum
C ) Stool
D ) Urine

As a side bar comment: if a patient has a pure culture of S. aureus/MRSA in their urine, they have a high likelihood of being septic with this organism. As a technologist, I always look at a patient's history in our department when I have a urine growing this bug. As a doctor, you guys should always be alerted to possible sepsis if you get a report from the lab showing a pure culture in the urine. Probably is a good idea to do some blood cultures if they haven't already been drawn.
