Hi,
I am not disappear this time. I read askdoc's posts, so I reread the section V. Thanks Focus for giving us this link. I got cold again, it's the forth time I got sick for this winter, and I have to take care of my daughter 24hrs a day and 7 days a week for next month. I know I should progress more, I'll try.

MA'M KG... indidoc reporting fr duty ma'm... precisely after 7 days of leave  
feels gooood to be back after so long.. lemme tell u all whats happened in this time..
Well, i had taken leave frm the forum bcoz i thought i would not be able to study continuously in this week n study will be frequently disrupted.. Thats exactly what happened.
I came back frm delhi today (my cousin's wedding, and i overstayed by 1 day )
Then i went(without resting) to take care of what has been my nemesis fr the past few days... my internship completion certificate... still haven't got it u'd have thought that after i got it typed nicely n got the 45 signatures needed(took me 2 days) the @#@#%#$ ^#@^@#% ^%@^^@%&&%&( clerk would have got it signed at least... Well, my cousin's started a new life, i have travelled 800 km in this week but that blasted %$&^& &^* has not even got one stamp on the damn piece of paper Told me to check with him tomorrow again. n here i was thinking i'd get all of this week to catch up with focus n mytime.
Anyhow.. in the midst of all this dancing n running around, i got 4 days of study... Did 6 chapters of neuro in these.. nearly 100 pags+lects so... did all right considering my state of affairs.
So much fr the past... now, in this week ..i plan to finish neuro in 2 days( starting today, if i can get my tired mind wheels rolling)
after that, i m gonna jump in n pick up whatever mytime n focus r doing frm physio+path+pharm. I may not do things as fast or as well as both of them r doing but i ll give it my best shot... n i guess 2 whips on my tail should get me going
i may also throw in 1/2 chapter of beh. science per day to make a nice muddle in my mind
All i know is that i have to get my 1st read over in feb itself.... the earlier... the better.

PLZ POST WHATEVER COMES TO UR MIND ABT THIS ONE.. 1ST THE DIFFERENTIAL DIAGNOSIS N THEN THE INVESTIGATIONS.. Plz heed my request.. it will help us all...
Here's the Q..there r 2 parts.. plz answer both with explanation..
1- A 40 yr old male body builder presents with fainting when he was lifting weights at the gym. There have already been 3 such episodes.The patient describes that he remained unconscious for about 3-4 minutes(according to his gym mates) and then regained consciousness without any residual symptoms. He denies presence of any breathlessness before or with the episode but reports a sudden,sharp pain shooting down his left upper arm just before fainting. There was no instance of vomitting or convulsions. On persistent questioning, he says he had earlier twice or thrice felt "ants crawling down my left upper arm and forearm (not hand) when exercising but its not much- probably muscle stretches due to weight lifting. " He does not smoke and is an occasional social drinker. He is not a diabetic (FBS=105) but has moderate hypertension. His father had "high cholesterol and died due to a heart attack".
Hb is 15g% and blood counts are normal.The fainting episodes are not linked to any particular posture of the head. S.cholesterol is 310mg/dl. Skull xrays and xrays of thoracic inlet show no abnormality.Treadmill test using Bruce protocol is within normal limits.
Given this information, the next best investigation to perform would be...
A) Electrocardiography
B) CT head
C) MRI head
D) Internal carotid arteriography in cranium
E) carotid arteriography in neck
F)Doppler duplex scanning of neck on left side
G) xray cervical spine
H) xray chest
I) echocardiography
J) arteriography of major aortic branches
K) MRI neck
L) CT neck
M) vertebral arteriography in cranium
N) nerve conduction tests on upper limb
O) vertebral arteriography in neck
P) Coronary angiography
And the investigation to confirm the diagnosis before initiating treatment would be-
A) Electrocardiography
B) CT head

indi, focus, zade, and everyone well, I said I was starting biochem, I only did half of one chapter (protein and aa chapter), been studying for the class that I am giving at med school, we are know reading about down syndrome, and fragile X syndrome etc, so even if it is not MLE, I think the info will stil help me for step 1.
Some of my students are interested in doing step 1 when they finish the basic semesters so I am trying to add info that is board relevant to the class.
I am relly enjoying teaching, it puts a smile in my face, and I am sorry that I can't say the same thing about my work in the health center, but I am taking a couple of weeks off in February to catch up in my reading for step 1.
My back, well, I have to loose weight, I have an unstable back (that is what the Xray said), I have hyperlordosis, 2 lumbar herniated disc, rectification of the sacrum, escoliosis, and degenerative changes, jejeje so, I guess in the future I might have surgery but for now I just need to focus on exercising (started doing stationary bycicle), and stop eating compulsively, jejeje.
By the way, when I study I feel the need to munch on something, do you guys feel the same?, and if so, what do you eat?
mytime, you are right, everyone is writting on the other thread, thank you so much for taking a look at this one, but if everyone agrees I can jump into the other one too, so that you don't have to read two threads and just come to this one for me
Happy studying and I am SURE that I will report more biochem chapters during the week, probably on Friday or Saturday will post again.

and to confirm MRI neck...

The Major Points which led me to the Answers were 2
1. Episodes were of not much longer period which suggest something like angina a short period of obstructed flow which is improved on rest.
2. Secondly he is a weight lifter his body is used to strain ofcousre his treadmill will be ok as they dont go to the limit but is still fainting so before diagnosis of an actual vascular event wud a Doppler or & Echo with an Arteriogram as a definitive
Now the part which bothered most of us was Tingling in the arm n numbness before the event. He is having Anginal pains possibly ?? We all jumped to neural reasons or obstructive pathology but it was a physiological answer any strain on the heart will as always in a non diabetic pt lead to a pain which looks like MI or Anginal or whtever pathology its so severe that he faints of course its vascular nothing neural abt it its a reflex action of the heart if u may say so. The reason i choose Echo n Doppler was that they both r non invasive n fairly more precise before we go into the invasive procedures
