A 25-year-old man comes to the emergency department because of a productive cough, fever,night sweats, shortness of breath, malaise, weight loss, and poor appetite that have progressively worsened over the past 3 weeks. He has worked in construction in the USA since emigrating from Thailand 1 year ago. His temperature is 38.3oC (101oF) and respirations are 30/min. He appears emaciated and has prominent needle tracks on his forearms. Which of the following is the most likely diagnosis?
A) Acute respiratory distress syndrome
B) Atypical viral pneumonia
C) Disseminated tuberculosis
D) Hypersensitivity pneumonitis
E) Metastatic carcinoma of the lung
F) Mycoplasmal pneumonia

i dont get the "needle track" referance

I would choose A

It seems that he's got some king of septicemia w/ S. aureus -> septic endocarditis & ADRS. It sounds better to me than TB, but this low temperature is bothering me (it doesn't fit the septicemia picture). One explination would be that he's got his defenses down
