If this episode is acute and he is not vitally stable then cardioversion, if he had chronic or duration of the disease is not know then anticoagulation and rate controll is recomended.(his bp can improve if u controll the ventricular rate.) and i want to make one correction also its atrial fibrillation not auricular .

never heard of atrial fib in pediatric patients (that is perhaps because Iīm not a pediatritian  ). What would be the medical management of this patient? I would go for... Anticoagulation for prohpylaxis of embolic complications. Digoxin, beta-blocker or verapamil for ventricular frequency control. Amiodarone for pharmacological cardioversion, when apropriate. If pharm cardioversion donīt work... electrical cardioversion. (How many Joules??)
