a 55 year old ,an is exposed to malathion while on a farm
after successful treatment of cholinergic symptoms with atropine,he continues to have difficulty
breathing and muscle weakness

2PAM is a reactivator of acetylccoline esterase -- this is the enzyme that is inhibited by organophosphates --e.g malathion

2PAM has to be given early in the treatment of organophosphate poisoning becausee the enzyme ages and this prevents reactivation

Why would we give an AChase re-activator in Myasthenia Gravis?

(we give AChase inhibitors...)

actually pralidoxime also blocks Acetyl cholineesterase reversibly. but i dont know wether it is given in myasthenia gravis...
