A 27-year-old woman comes to the phisician because of a 3-weeks history of fever, night sweats, rash on both legs, nonproductive cough, and pain and swelling in her wrists and knees. She has not had weight loss. Her temperature is 37.7 C(99.8 F), blood pressure is 110/70 mm Hg, pulse is 96/min, and respirations are 14/min. The lungs are clear to auscultation. Cardiac examination shows no abnormalities. There is swelling and warmth over the wrists and knees bilaterally and tender red nodules on the anterior surface of both lower extremities.
