17. A 24-year old man presents to the ED with hypertension, tachycardia, an elevated body temperature, diaphoresis, mydriasis, and severe agitation. His mother reports that he uses illicit drugs although she is not sure which kind. Which of the following agents is the most appropriate therapy for this patient?

A. Atropine
B. Flumazenil
C. Fluoxatine
D. Labetalol
E. Naloxone
F. Physostigmine


Which drug was he taking? and what is the mechanism of the therapy?

it sounds like anticholinergic "jimson weed"
so atropine is out

after the patient arrives to ER
you should do a gastric lavage "pump the stomach" with activated charcoal
and use physostigmine

Physostigmine is indicated in atropine overdose,overdose of phenothiazines and TCA'S...SO better choice here is F..

illicit drug would be amphetamine -in the real world supportive treatment, but since you ask physostigmine.
[usmle will give more clues because amphetamine works directly at the synaptic cleft and cocaine and TCA's work indirectly at the synaptic cleft.]

THE RIGHT ANSWER IS D.

The patient described is under the influence of a CNS stimulant, such as methamphetamine. Labetalol is a nonselective alpha and beta-antagonist that blocks many of the dangerous peripheral side effects of CNS stimulants, such as hypertension and cardiac stimulation. Other appropriate medications include antipsychotic agents (to control the agitation and psychotic symptoms) and diazepam (to control possible seizures). Supportive care should be given as needed to control the hyperthermia and to maintain respiration.
