A 31 year old man comes to the physician becacuse of decreased libido
He had viral orchitis 6 months ago
physical examination shows gynecomastia and testicular atrophy
which of the following drugs is most appropiate for long term management of this condition ?

a.-danazol
b.-gonadotropin releasing hormone
c.-human chorionic gonadotropin
d.-sildenafil
e.-testosterone

Viral orchitis is usually unilateral in about 70% of cases,
The question didn't say if the testicular atrophy was unilateral or bilateral
if unilateral, then GnRH would help... If bilateral, then probably Testosterone is a better choice.

testosterone would be better choice , GnRH can excerbate gynaecomastia

A GnRH analog can work differently if given pulsatile vs. continuous.

Given in a pulsatile manner it will INCREASE FSH and LH and thus increase sex steroids

Given in a continuous manner it will DECREASE FSH and LH and thus decrease sex steroids

I think the question should have been more specific! but anyway, Testosterone is still a better choice because since there is testicular atrophy, you can't stimulate the testes to produce testosterone.

yes virgo. because of atrophy you need to prescribe testosterone .
