Which antihypertensives are first line for blood pressure control in the face of diabetic nephropathy with azotemia?

thanks dr in tr..., but i was thinking, azotemia indicated toward development of renal insufficiency, thus would be a contraindication for ACE-I use.

What would be the alternative to ACE-I in such a case? ARII-Blockers or..?

First of all, ACEI is used in Type I diabetics for reduction of mortality.

ARB is used for Type II Diabetics.

The goal is 130/80.

HCTZ is contradicated in azotemia patient.

Azotemia: Azotemia may be precipitated or increased by the hydrochlorothiazide component of Micardis Plus (ARB). Cumulative effects of the drug may develop in patients with impaired renal function since the primary route of excretion is through the urine.

is there a difference in Tx of HTN in DM I & DM II about ACEIs?
Could u clear this doubt, dr AAAAA?
