A previously healthy 65 year old man presents at the ER due to 1 hour of chest pain. His pulse is 95, BP 160/110. His lungs are clear to auscultation. He has an S4 gallop and a grade 2/6 systolic murmur at apex. An ECG lead shows 3 mm of ST segment elevation in lead V2-V5. The most appropriate immediate treatment is:
A. captopril
B. furosemide
C. tPA
D. magnesium
E. digoxin

you dont want to think too long or delay tPA!!

captopril- the ace inhibitor blocks angiotensinII/// and brady kinin is responsible for the infamous cough.

furosemide-famous for inhibiting the Na Cl K cotransporter at the ascending loop. causes loss of many lytes and can be used in hypercalcemia "Lovely Lasix" "high ceiling"
{ "give me some Lasix"} "sulphur moeity"

Mg an essential lyte that is used in preeclampsia

Dig {from the foxgloveplant}-world known for inh. the Na/K pump then getting a flux of Ca. used in heart failure, works at AV node causing a + ionotropic affect and negative chronotropic affect. Known for toxicity and dig levels.

why is there a systolic murmer at apex?
why s4 gallop when lung bases are clear cant be heart failure!
