ive been offered a pgy1 position in IM at a community hospital with no inhouse fellowships. Im a 1999 graduate ,us citizen with scores of 92 and 88 in step 1 and 2 both first attempts and cs passed on 2 nd attempt ,no USCE just one month observership in pathology but dont intend to go for pathology since i like IM better .i also want to do fellowship after graduation.should i accept the offer or wait till next year in hopes of getting matched in a university program?

do the residents there go on to fellowships?? If so take the position. If it is a malignant comunity program I might reconsider, but you are saving a year; and usually doing a chief resident year (an extra pgy-4 year) will get you a fellowship... so I would take the position unless it is one of the absolute worst programs out there (ie Harlem, or on probation, etc..)

thanks so much for the replies. some of its graduates do go on for fellowships and its not malignant either . can u explain to me a bit more about the chief resident year in regards to the fellowship?how it helps for the felloship, is it considered as a fellowship year 1 and wat u do in that year or do u apply for fellowship after completing that year ?

I don't know too much about it. All I can say is that it is an extra year (pgy-4)which is pretty much like your third year but you are also chief resident. It doesn't count towards fellowship, but you have a greater chance of getting a fellowship afterwards. One of my interviewers at a lesser known community program was doing this in order to get a cardiology fellowship, and while I don't know if he got it, he did tell me he was getting interviews. The reason I mentioned this is that even if you were in an awful program where noone got a fellowship, a chief resident year would get you a fellowship somewhere... but seeing as residents do go on from pgy-3, i don't think you will have a problem. TAKE THE PREMATCH!!!

most comm progs dont have an extra chief yr.. the pgy3 is just designated as chief.. the duties arent significant as a uni pgy4 chief.. just makin the schedule and stuff.. but does add weight when u go for fship.. espec the lower ranked ones.. for cards and gi its less effective unless u have a stellar all round cv.. for last yr the chief of even uconn didnt make it to gi.. apurva

is the pgy4 chief resident year different from pgy4 fellowship year?
